Getting Started...

Joining our Practice is simple:

All you have to do is book a new patient examination.

All new patients are requested to arrive 15 minutes in advance of their appointment . Upon arrival, you will be required to complete several forms including:

  • Patient Details

  • Medical History

  • Oral Health Survey

  • Contact Consent Form

New Patient Examination...

​

All new patients are advised to leave a minimum of 45 minutes for their first examination to allow for a full assessment and to have the opportunity to discuss any concerns.

​

A treatment plan will be created if needed.
